SUBJECT:- Provide information under section 6(1) and 6(3) of Right to Information Act 2005.

Sir/Madam,

I am resident of (your full address i.e House no. xxx, colony/sector yy, city). My wife( name of your wife) had given a complaint (complaint number xxxx, Dated zz-zz-zzzz) in women cell(address where your wife had given complaint)which is related to your department/office. Kindly provide me the following information under right to information act 2005:-

(1) Provide certified copy of complaint which was given by my wife(name of your wife) in women cell(address where your wife had given complaint).

(2) Provide certified copies of all documents which were attached with the complaint given by my wife(name of your wife).

(3) Provide certified copy of attendance sheet where me and my wife signed for the presence/appearance.

(4) Provide certified copies of lists of stridhan which were given by my wife(name of your wife).

(5) Provide certified copies of all the documents which were given by me in this case/complaint.

(6) Provide certified copies of all written statement/replies which were given by all the respondents including me.

(7) Provide certified copies of all written statements/replies given by the complainants or my wife named(name of your wife) in above mentioned complaint.

(8) Provide certified copy of last investigation report or any other reports which were made by investigation officer or concerned officer.

(9) Provide certified copies of all any other documents which are related with the complaint/case.

If all above information is not related with your office/department then please forward this request/application to the concerned office/department under section 6(3) of RTI Act 2005 and also please CC to me, incase if they donot provide me the response, I shall be able to file First Appeal to the concerned department.
